No, we take walk-ins, as well as appointments.

Where can I obtain the request for Livescan service form?

APPLICANT TYPE VARIES ON:

Employment: The employer provides this form
License/Permit: The Board, Bureau, or Department provides this form.
Personal/Immigration: Attorney/Lawyer should advise which Live Scan form is necessary!
Volunteer: Agency's location of volunteer work.
Community Care Licensing: Employer's agency must provide form.
All other purposes should be provided by Requesting Agencies.

Valid Identification Card (no expired ID) Must have a photo, expiration date, and basic information
